ABSTRACT

@#Along with the popularity of low-dose computed tomography lung cancer screening, an increasing number of early-stage lung cancers are detected. Radical lobectomy with systematic nodal dissection (SND) remains the standard-of-care for operable lung cancer patients. However, whether SND should be performed on non-metastatic lymph nodes remains controversy. Unnecessary lymph node dissection can increase the difficulty of surgery while also causing additional surgical damage. In addition, non-metastatic lymph nodes have been recently reported to play a key role in immunotherapy. How to reduce the surgical damage of mediastinal lymph node dissection for early-stage lung cancer patients is pivotal for modern concept of "minimally invasive surgery for lung cancer 3.0". The selective mediastinal lymph node dissection strategy aims to dissect lymph nodes with tumor metastasis while preserving normal mediastinal lymph nodes. Previous studies have shown that combination of specific tumor segment site, radiology and intraoperative frozen pathology characteristics can accurately predict the pattern of mediastinal lymph node metastasis. The personalized selective mediastinal lymph node dissection strategy formed from this has been successfully validated in a recent prospective clinical trial, providing an important basis for early-stage lung cancer patients to receive more personalized selective lymph node dissection with "precision surgery" strategies.

With the rapid development of biliary and pancreatic endoscopy, it has become an important vehicle for the diagnosis and treatment of complex biliary and pancreatic diseases. In recent years, the authors have combined accurate endoscopy with precision surgery in the treatment of clinical complex biliary and pancreatic diseases, achieving good effects.

ABSTRACT

Benign biliary strictures (BBSs) is a kind of difficult clinical problem in biliary surgery. Surgery and endoscopic treatment are common diagnostic and therapeutic methods. The rapid development of endoscopic technology challenges the traditional surgery. How to integrate surgery and endoscopic technology in an orderly manner and develop strengths and circumvent weaknesses requires a breakthrough in guiding ideas. The precision surgery, with the characteristics of certainty, predictability, controllability, standardization, individualization and systematization, has become an ideal choice for integrating surgery and endoscopic technology. Based on clinical practice, the authors discuss the endoscopic diagnosis and treatment strategy of BBSs from the perspective of precision surgery by implementing the core elements of precision surgery.

ABSTRACT

Objective:To analyze and summarize the application and significance of the computer-assisted surgery system in precision surgery of mediastinal tumors in children.Methods:The clinical data of 54 children that underwent mediastinal tumor resection surgery in the Affiliated Hospital of Qingdao University from August 2008 to November 2021 were collected.According to whether the Hisense CAS computer-assisted surgery system was used before the operation, the patients were divided into three-dimensional (3D) reconstruction group (29 cases, 53.70%) and two-dimensional (2D) CT group (25 cases, 46.30%). The surgical indicators, postoperative hospital stay, and the incidence of surgical complications were compared between the two groups.The t-test, Mann- Whitney U test, χ2 test or Fisher′ s exact test were carried out for statistical analysis.Simulated surgery and actual surgery were compared for both 3D reconstruction and 2D CT groups. Results:There were no significant differences in gender, age, height, weight, maximum tumor diameter and tumor location between the 3D reconstruction group and 2D CT group (all P>0.05). The operation time of the 3D reconstruction group was [(125.14±41.37) min] was shorter than that of the 2D CT group [(149.24±44.53) min] ( P=0.044). The intraoperative blood loss in the 3D reconstruction group [15.00(13.50, 25.00) mL] was less than that in the 2D CT group [36.00(30.00, 75.00) mL] ( P<0.001). In addition, the indwelling time the closed thoracic drainage tube [4.00 (3.00, 5.50) d] and postoperative hospitalization days [(8.83±3.39) d] in the 3D reconstruction group were shorter than those in the 2D CT group [7.00(5.00, 11.50) d, (11.00±4.10) d] ( P=0.001, 0.038). No significant difference in postoperative complication rates was found between the 3D reconstruction group and 2D CT group ( P>0.05). The simulated operation was consistent with the actual operation in the 3D reconstruction group. Conclusions:3D reconstruction by the computer-assisted surgery system can truly reveal the anatomical relationship between tumors and surrounding organs and blood vessels, and improve the accuracy and safety of surgical resection of mediastinal tumors in children.

ABSTRACT

The concept of precision medicine has brought revolutionary changes to human health and medical treatment. In a broad sense, precision medicine is the application of advanced technologies such as gene sequencing, digital medicine, artificial intelligence, navigation technology, and biomedical engineering to clinical, basic, and public health medical research and practice. The purpose is to promote the continuous development of medical standards. The concept of precision surgery was first proposed by academician Dong Jiahong, who has played an exemplary role in the field of liver surgery for the oncology and other professional fields of surgery. The development and application of computer-assisted surgery systems have promoted the development of precision liver surgery. The digital liver three-dimensional reconstruction provides a new basis for liver vascular anatomy and liver segmentation, assists in the realization of individualized surgical planning, and the measurement of residual liver volume guarantees the safety of operation. The great progress of adult precision liver surgery has provided important help and guidance for the development of pediatric liver surgery. Compared with adults, pediatric liver tumors have their particularities. Pediatric liver tumors are often huge, complex in location, fast growing, and highly malignant. Most children do not have liver cirrhosis and are sensitive to chemotherapy. The liver volume changes greatly with age and weight. These differences determine that the treatment of liver tumors in children needs to adopt different models, and the diagnosis and treatment standards need to be improved. The authors elaborate on the topic of "from adult to child-the development and prospect of precision liver surgery" to promote precision surgery and comprehensive treatment of liver tumors.

ABSTRACT

OBJECTIVE: To explore the clinical value of three-dimensional visualization technology in the resection of retroperitoneal tumor.METHODS: A retrospective analysis was performed on 91 patients with retroperitoneal tumor who underwent surgical resection in Department of General Surgery,Affiliated Sixth People's Hospital of Shanghai Jiao Tong University from January 2014 to December 2018.The three-dimensional visualization technology was used to observe the relationship between retroperitoneal tumors and surrounding tissues and blood vessels,to assess the resectability of the tumor and guide the development of the surgical plan.RESULTS: The median operation time was 178 minutes,the median blood loss was 543 mL,and the average tumor size was 12.8 cm.Nine cases(9.9%) were with postoperative complications and 1 died of seizures,82 patients with pathological examination of tumor capsule integrity,and the rate of complete resection is 90%.CONCLUSION: Preoperative application of three-dimensional visualization technique in the resection of retroperitoneal tumor can improve preoperative assessment accuracy and guide the formulation of individualized precision surgical procedures,meanwhile,to enable patients to achieve better surgical results.

ABSTRACT

Perihilar biliary tract tumours include hilar cholangiocarcinoma, gallbladder cancer invading the hepatic hilum and intrahepatic cholangiocarcioma invading the hepatic hilum.The tumours have the special characteristics such as strong invasion capability, the anatomic variant, the pathophysiological complexity, the biological behavior diversity and the difficulty of preoperative evaluation which result in low R0 resection rate, more postoperative complications, more mortality and poor prognosis.The perihilar surgical techniques system aims to set a reasonable and individual operation procedure on the principle of precision surgery by taking the key technique of hilar dissection and exposing, evaluting the hilar anatomy by the means of three-dimensional digital medical image evaluation system, evaluting the relationship between the tumour and hepatic artery and portal vein under the condition of hepatic hilum complete exposure by combining anterograde and retrograde route from intra and extra-hepatic direction to hepatic hilum.The perihilar surgical techniques system is applied to improve the accuracy of resectable evaluation, the R0 radical resection rate, the safety of operation and the accuracy of cholangiojejunostomy through the accurate preoperative evaluation, the detailed operation plan, the accurate intraoperative reassessment, the perfect operative procedure and the excellent postoperative management, eventually for the purpose of reducing the postoperative complications rate and perioperative mortality and improving the prognosis of perihilar biliary tract tumours.

With the proposal of the concept of precision surgery,doctors pay more attention to the precious balance of removing target lesions,protecting organs,and controlling damage.The application of intraoperative computed tomography (ICT) facilitates the realization of precision surgery.This paper summarizes the origination and development of ICT,as well as its wide applications in neurosurgery,otolaryngology,and maxillofacial surgery.Although ICT has many disadvantages to be improved,such as high radiation dose and high cost,it is still a good choice for improving the precision of treatment.

ABSTRACT

Due to the theoretical and technique limitation of traditional surgery,surgical treatment of complex intrahepatic biliary diseases was left for an unresolved difficult problem of the last century.Uncertainties of the anatomical,physiological,pathological changes and surgical techniques in the intrahepatic biliary tract contribute to this complexity.Through integrated application of modern scientific technology and traditional medicine methods and systematic optimization and innovation of biliary surgical theories and techniques,authors have developed a paradigm of precision biliary surgery which is characterized by high quantification,visualization and controllability.The establishment of the precise biliary surgical system solves the difficulty in surgeries for intrahepatic biliary diseases,puts forward the biliary surgery from extrahepatic ducts,hilar ducts to intrahepatic ducts,entering a new segment era of the biliary surgery marked by precision treatment of intrahepatic biliary diseases.

ABSTRACT

Reoperation on biliary tract diseases is a kind of complex and difficult surgery,with the high risk and recurrence rate.Benign biliary tract diseases included residual and recurrence of bile duct stone,bile duct injury and benign biliary stricture,which are still the most common causes for reoperation.The common causes,application of the precision surgery and new mode of multidisciplinary team (MDT) for the reoperation on benign biliary diseases were explored in this paper.With the techniques developments of minimal invasive and precision surgery,the new mode of MDT for the reoperation on benign biliary diseases will provide the best individual diagnosis and treatment to the patients.

ABSTRACT

Objective:To investigate the significance of 3D printing technology in the treatment of skull base tumor.Methods: A 45-year-old male patient who felt headache and discomfort had progressive loss of vision to blindness in the left eye. Using MR plain scan found that there was a skull base tumor, so the patient was examined by brain CTA and 3.0T MR to obtain medical digital imaging of CT and MR and DICOM data which were input into computer for establishing three-dimensional model, and then printed it.Results: 3D reconstruction model can clearly display the anatomical structure of skull base tumor, three-dimensional space and peripheral blood around skull base tumor. Under the guidance of this new technology, the tumor was completely removed and the surrounding normal brain tissue was preserved as much as possible. The whole operation time was greatly reduced at the same time.Conclusion: The treatment of 3D printing technology is applied in skull base tumors can clearly display the anatomical structure, distinguish adjacent vessels around tumor; when it avoids missing tumor, also can more effectively assist preoperative planning, enhance the security for complex skull base surgery, accelerate the operation speed, improve operation accuracy; it has great application prospect in the precise treatment of neurosurgery.

ABSTRACT

Objective To study the assisted diagnostic value and significance in precise surgery of MSCT angiography in torsion of ovar-ian tumors in children. Methods The dual-phase enhanced MSCT images data of 15 children patients with ovarian tumor torsion which were confirmed by surgery were retrospectively analyzed. Showed the vascular changes around the ovaries and judged the ovarian tumors arise and torsion side with the vascular reconstruction methods of multi-planar reconstruction (MPR),maximum intensity projection (MIP),volume rendering ( VR) ,then compared with the operation situation. Results Among the 15 cases aged from 2 months to 11 years old who were de-tected with MSCT scanning and angiography, there were 11 cases of benign tumor (9 cases of ovarian teratoma and 2 cases of serous cystade-noma) and 4 cases of malignant tumor (3 cases of yolk sac tumor and 1 case of malignant mixed germ cell tumor). MSCTA displayed that o-varian vessels changes were abnormal thickening and twisting of distal feeding artery and tumor vascular network formation;and there were thickening, tortuosity, increased density and a few visible‘whirl sign’ in draining veins. Preoperative MSCTA displayed ovarian tumor blood supply arteries were found 5 cases of downlink of ovarian artery and 10 cases of asscending branches of uterine artery. There were 4 cases of ovarian tumor originated from the left ovary combined with reverse and 11 cases originated from the right ovary combined with reverse, which were consistent with intraoperative findings. Intraoperative MSCTA provide information which helped accurate chose of the surgical incision, precise abruption of tumor blood vessels and resection of the tumor and ovarian lesions, thus saved the operation time and reduced the blood loss. Conclusion MSCT angiography can not only provide diagnosis information of torsion of ovarian tumors in children,but also more impor-tantly,it has an important assisted significance in precision surgery.

ABSTRACT

Esophageal variceal hemorrhage is a severe complication of portal hypertension.Non selective-blockers and endoscopic variceal ligation have been utilized to prevent variceal hemorrhage as the first line therapeutic measures.The role of surgical approaches in the treatment of portal hypertension is still controversy.The selective shunt operation,devascularization operation,transjugular intrahepatic portosystemic shunt and liver transplantation have their own indications and limitations.Based on the precision surgical therapy,individualized selection of suitable surgical management on portal hypertension should be advocated to minimize invasiveness and cost-effect ration,maximize organ saving and outcome.For patients with extrahepatic portal hypertension,Meso-Rex bypass operation could be the preventive treatment method,surgical treatment could be applied to patients with regional portal hypertension.
